My CS-Link - Sign Up

We need some information from you in order to grant you a My CS-Link account. Once submitted, it could take up a week for processing and verification. Once approved, you will receive an email or a letter with your activation code and instructions on how to activate your My CS-Link account. If you have any questions, please contact your clinic.

If you are requesting access to another person’s health information, please complete a Proxy Access and Authorization Form. Huntington Health patients should use this form to request proxy access. Access to another person’s health information in My CS-Link cannot be granted online at this time.

Once your identity is verified, you will receive a confirmation email with instructions on how to activate your My CS-Link account.
